There's a metal end cap that goes on the end of the actual handlebars. Handlebars are made from tubed metal and are generally hollow so the end cap keeps water from getting in there.
My guess is, for whatever reason, someone changed the handle grips and the old end cap got stuck in the old one. Now instead of a smooth end there's a sharp edge, which has now cut/pushed through the end of the new grip.
I forgot what kind of bike you said you had but search for "bike" "end caps" and you should find them. Then all you need is a new set of handlebar grips.
